dc.description.abstract | Šī darba ietvaros tika pētīta 6. herpesvīrusa (HHV-6) infekcijas iespējamā loma autoimūnā tireoidīta (AIT) izraisīšanā. Pētījumā iesaistīti 52 pacienti ar AIT, kuriem veikta tireoīdektomijas (Thyreoidectomia totalis) operācija, 30 autopsiju paraugi un 30 veseli asins donori kā kontroles grupa. DNS no asinīm un audiem tika izdalīts, izmatojot fenola- hloroforma metodi. HHV-6 vīrusu genoma secības klātbūtne asinīs un audos noteica ar polimerāzes ķēdes reakciju ar iekšējo praimerēšanu (nPCR). PCR produkti tika analizēti ar elektroforēzi agarozes gēlā. Lai apstiprinātu iegūtos nPCR rezultātus, plazmas paraugos tika noteiktas HHV-6 specifiskās IgG klases antivielas, izmantojot komerciālo ELISA reaģentu komplektu. Lai noteiktu autoantivielas pret tireoglobulīnu (TG), tireoperoksidāzi (TPO) un TSH receptoriem, izmantoja ELISA reaģentu komplektus. HHV-6 pozitīvajiem paraugiem veica vīrusa slodzes noteikšanu ar HHV-6 Real-Time reaģentu komplektu. Histoloģiskos preparātus analizēja ar gaismas mikroskopu. Veicot ELISA testus pret tireoglobulīnu (TG), tireoperoksidāzi (TPO) un TSH receptoriem, visiem pacientiem ar AIT tie bija pozitīvi, savukārt nevienam no veseliem donoriem neuzrādījās autoantivielas pret TPO, TG un TSH receptoriem. Salīdzinot ar kontroles grupu, persistentas HHV-6 infekcijas klātbūtne pacientiem ar AIT bija ievērojami lielāka (p=0.0031), attiecīgi 69% un 31%. HHV-6 U79/80 mRNS sastopamības biežums AIT vairogdziedzera audu paraugos bija ievērojami lielāks (24/51 (47%) salīdzinot ar 1/17 (6%); p=0.0028) kontroles grupu. Vidējā HHV-6 vīrusa slodze, salīdzinot aktīvas un latentas infekcijas pacientus, bija augstāka pacientiem ar aktīvu vīrusa infekciju (2147 IQR: 971-4188 pret 551 IQR: 145-1589 kopijas/106 šūnām). Visos paraugos detektēja HHV-6B vīrusa secību. Darba saturu veido 5 nodaļas ar apakšnodaļām. Kopumā darba izstrādāšanai izmantoti 44 literatūras avoti, 16 attēli un 2 tabulas. dc.description.abstract | The aim of this study was to evaluate the potential role of human herpesvirus 6 (HHV-6) infection as a trigger of AIT disease. This study included 52 AIT patients, 30 autopsied subjects without thyroid pathologies and 30 healthy donors as controls. DNA was extracted from peripheral blood by the phenol-chloroform method, followed by nested PCR for detection of HHV-6. The PCR products were observed by electrophoresis in agrose gel. To confirm nPCR results, antibody responses against anti-HHV-6 IgG antibody were tested using ELISA kits. ELISA kits were also used to determine autoantibodies against TPO, TG and TSH receptors. Based on PCR product size and sequencing, the HHV-6 A or B subtypes were characterized. Viral load was analyzed using quantitative real-time polymerase chain reaction. Histological samples were observed and analyzed using light microscopy. Auto-antibodies against TPO, TG and THSr in various combinations were detected in all 52 patients with AIT. Mentioned auto-antibodies were not detected in any of healthy blood donors. The presence of persistent HHV-6 infection in AIT patients was significantly higher (69% vs 31%, respectively, p=0.0031) than in the control group. Also, significantly higher frequency of HHV-6 activation marker (U79/80 mRNA) was found in patients thyroid gland tissue samples with AIT in comparison with control group (24/51 (47%) vs. 1/17 (6%), respectively; p=0.0028). Median HHV-6 load was found higher in patients with active viral infection than in patients without it (2147 IQR: 971-4188 vs. 551 IQR: 145-1589 copies/1x106 cells). HHV-6B type was detected in all samples. The thesis has 5 chapters with subdivisions. The total amount of 44 references, 16 figures and 2 graphic charts are used in this thesis.
