Imūnas atbildes marķieru saistība ar HLA II klases alēlēm akūtas SARS-CoV2 infekcijas gadījumā

Abstract

SARS-CoV-2 vīruss cilvēka organismā spēj izraisīt dažāda smaguma Covid-19 slimību. Viens no faktoriem, kas nosaka imūnreakcijas īpatnības un, tādejādi, spēj ietekmēt slimības norisi, ir HLA gēni. Darbā tika pētīta saistība starp imūnatbildes marķieriem, HLA II klases alēlēm un slimības smagumu 77 pacientu grupai ar pirmreizējo Covid-19. Pacientiem ar smagāku slimības norisi genotipā divreiz biežāk tika konstatēta HLA-DRB1*14 alēle, bet iestājoties stacionārā, tiem bija zemāks absolūtais limfocītu un CD4+ T limfocītu skaits, kā arī bija augstāki IL-6, IFN-gamma un CRP līmeņi nekā pacientiem ar vidēji smagu norisi. Turklāt HLA-DRB1* 03; 07, un 15 alēlēm, HLA-DQA1* 04:01 alēlei un HLA-DQB1* 02:01-02; 03:01; 03:02; 04:01-02; 05:01 un 06:01 alēlēm bija konstatēta saistība gan ar augstākiem, gan zemākiem imūnatbildes marķieru līmeņiem.

SARS-CoV-2 virus can cause COVID-19 disease of varying severity. One of the factors that determines the characteristics of the immune response and, therefore, can influence the course of the disease, is HLA genes. Bachelor thesis investigated the relationship between immune response markers, HLA class II alleles, and disease severity in a group of 77 patients with first-time COVID-19. Patients with severe/critical disease compared to non-severe patients had lower absolute lymphocyte and CD4+ T lymphocyte counts and higher levels of IL-6, IFN-gamma and CRP. Also, the HLA-DRB1*14 allele was detected twice as often in the severe patients genotype. In addition, HLA-DRB1* 03; 07, and 15 alleles, HLA-DQA1* 04:01 allele and HLA-DQB1* 02:01-02; 03:01; 03:02; 04:01-02; 05:01 and 06:01 alleles were found to be associated with both higher and lower levels of immune response markers.
